My clan runs two "tactical realism" COD4 servers that your welcome to try.
One is ranked high in the top 100 for volume of players and the other has a custom modification created by one of our members.

The RTR mod features
You bleed out when shot but you can apply bandages to yourself to heal. You can also apply bandages to wounded teammates.
No run and gun or shooting from the hip (must use weapon sights when firing)
No bunny hopping
No hit crosses and no kill messages. The only way to confirm you killed someone is to see the body.
No nade or m203 spam.
limited weapon availability (only X amount of m203's are allowed) and you must select your weapons at the start of a new map.
Claymores have no "laser" or "icon" indicators and can kill your teammates and yourself if you walk in front of them.
Jumping down from an unrealistic height can injure you and cause you to drop your weapon.
No bomb plant icons or count down timers. The bomb plant is randomized as it the attacking and defending team.
Excessive firing of your weapon can lead to a gun jam or double feed which you have to clear before using that weapon again.
We are also in the process of creating new custom maps.
